PLEASE NOTE: that as of July 26th, 2026 at 1:00 PM EST that should this form be used as form of solicitation (meaning if you submit ANYTHING trying to sell me something or get me to use your product, this includes emails about SEO/loans/software/hardware/website hosting or anything you are trying to sell that I did not contact you about first) that a $100 invoice will be submitted to the email provided and you agree to pay within 14 calendar days, a late fee of $50 will be added on for every 7 calendar days after that. Should you not pay after 60 days then I shall peruse legal action to receive payment. Where you are agreeing that the state of FLORIDA shall hold jurisdiction and you will be responsible for costs related to pursuing payment. This form is NOT for any solicitation at all.
